2023年7月12日,安徽医科大学第一附属医院张成鑫教授团队在成都微创心血管外科大会暨第七届西部瓣膜论坛期间,应大会邀请成功完成了2台经心尖TAVR手术,其中1台进行了在线直播,这也是当天手术直播活动中唯一一台经心尖一站式TAVR+TEER手术。
直播过程中,阜外华中心血管病医院葛振伟教授、中国人民解放军总医院第一医学中心姜胜利教授、四川大学华西医院赁可教授、武汉大学中南医院刘金平教授、南京鼓楼医院王东进教授等在线主持,共同参与讨论。

这两个病例各有特点,第一例为二尖瓣和主动脉瓣成形手术后合并横位心,第二例为重度主动脉瓣反流伴二尖瓣器质性病变。应对不同病例,张成鑫教授团队如何选择合适的治疗方法?团队做出了哪些努力确保患者得到最佳治疗结果?

病例一:二尖瓣、主动脉瓣成形术后+横位心
病例简介
该患者为老年男性。
心脏超声提示:二尖瓣成形+主动脉瓣成形+三尖瓣成形术后,主动脉关闭不全伴重度瓣口反流,肺动脉高压(轻中度)。
CT分析结果显示:患者主动脉瓣三叶式,瓣叶基本等大,瓣叶稍厚,无冠瓣瓣叶游离缘轻微钙化;主动脉瓣环周长折算直径约26.2mm;患者双侧冠脉开口高度可LCA:18.6mm,RCA:17.2mm。患者术中建议造影角度RAO:8°,CAU:10°;主动脉弓、降主动脉及右髂总动脉散在钙化;二尖瓣位见金属环。

手术难点
该病例最大的特点为患者重度主动脉瓣反流;横位心,瓣环夹角58°,心尖夹角155°;此外患者曾做过二尖瓣成形、主动脉瓣成形手术,相较于传统开胸瓣膜置换手术,适合选择经心尖途径的TAVR。

病例二:主动脉瓣关闭不全伴二尖瓣后叶腱索断裂
病例简介
患者为老年女性。
心脏超声显示:主动脉瓣关闭不全伴重度瓣口反流,二尖瓣后叶部分腱索断裂,二尖瓣后叶广泛脱垂伴重度瓣口返流。
CT评估:瓣环:27.9mm;左冠高度:13.4mm;右冠高度:20.6mm;主动脉瓣环夹角:65°;心尖入路角度:133°;心尖厚度:6.7mm。

手术难点
该病例最大的特点为患者重度主动脉瓣反流伴二尖瓣器质性病变导致的二尖瓣反流;横位心,瓣环夹角65°,患者心尖夹角133°。对于主动脉瓣关闭不全伴二尖瓣后叶腱索断裂的病例,适合进行一站式手术,考虑优先进行TAVR是因为主动脉瓣反流解除后可能在一定程度上能够缓解二尖瓣的反流,增强心脏的耐受性。
